How to Submit a Request:

  • Click Create at the top of the page

  • Fill in the Fields & Click Create:

Division Needed

Senior, Healthcare Select, SelectRx, Term Life, LHA/FE, SQT, SQAH

Support Team Needed

Application Support

Type of Request

Description

Provide details on the Request

  • Name of person(s) needing access

    • Their Emails

  • What environment(s) do they need access to?

    • Prod

    • UAT

    • DEV

  • What Role setup do they need?

    • If unsure, is there someone that can be used as a mirror for their permissions?

  • BPO Review and Labeling

    • Application Support SQIB Tickets will receive "Application Support" in "Support Team Needed" and will receive a notification when created.

    • Reference the Application Support Escalation Guide for escalation to CRM specific Team Lead.
