To submit and view pending requests navigate to the URL:

How to Submit a Request:

  • Click Create at the top of the page

  • Fill in the Fields & Click Create:

Division Needed

Senior, Healthcare Select, SelectRx (SmartSheet), Term Life, LHA/FE, SQT, SQAH

Provide a Title of the Request

Description

Provide details on the Request

  • BPO Review and Labeling

    • Application Support SQIB Tickets will receive "Application Support" in "Support Team Needed" and will receive a notification when created.

    • Reference the Application Support Escalation Guide for escalation to CRM specific Team Lead.
