These types of requests are to be submitted if a change is needed regarding the following:
Facility prioritization needs to be updated for a specific state(s) or carriers
Eligibility changes are needed for a specific state/facility
Enrollment caps need to be adjusted for a specific state/facility combo
Information needed for each request:
State(s) needing to be adjusted
Carrier/State combo needing to be adjusted
Requests need to be submitted by the following people or approval from them is to be requested/documented on the submitted ticket

Navigate to the SRx Ideas Board and click the "Create" button at the top of the page:
Fill out each request with these parameters
