Fingerprints or Other Licensing Documentation

Why are these documents important and how do I obtain them?

  • Some states require additional documentation to submit with an application. If the licensing team does not have these documents, an application cannot be submitted and the agent will not receive that state license. CA and FL require fingerprints to be submitted with each application. Each state will require 2 fingerprint cards. A local police station or UPS store should be able to provide an agent with FD 258 cards needed for fingerprinting. Georgia requires agents to complete a Georgia Affidavit form and submit a front and back copy of their Driver’s License/ID or passport for citizenship verification. This document is needed in order to obtain a license in the state of Georgia and must be notarized. Agents are sent a GA form during the onboarding process or an agent will receive one from the licensing team to be returned when needed.
